Vulnerable Adult Program Manager

The Vulnerable Adult Program Manager will work with vulnerable adults who require case management services, providing critical support services to vulnerable adults. The program manager will coordinate care in a client-centered manner, focusing on individual strengths, and the needs of the client. Conducting risk assessment and crisis intervention. The program manager will be the contact for Adult Protective Services when referrals are received, and be the APS Investigator on referrals.  This position will work with other service providers to ensure wrap around services are provided, these providers include but are not limited to: health center staff, Area on Aging Staff, CDWA, elders staff and other collateral service providers.  The program manager will also assist the Elders Program Manager with planning events/activities that will benefit all Elders in the Community. This position will supervise the Vulnerable Adult Program Assistant and the Special Needs Case Manager.

 

This position requires critical thinking and the ability to make sound decisions in stressful situations.

A thorough knowledge of the tribal code, court procedures, SSI and SSDI applications and processes, and protective payee knowledge.

This position requires a thorough knowledge of the tribal community, and the needs of their most vulnerable citizens.

This position deals with unique, complex situations, involving the most vulnerable tribal citizens.
